IN-HOME FUNCTIONAL ASSESSMENT

Functional Daily Living Assessment (Home Visit) - Home Care Assessment

An occupational therapist will assess the client’s ability to perform his/her Activities of Daily Living (ADLs) including self-care tasks and instrumental ADLs taking into consideration the client’s physical and cognitive abilities and current injuries as well as his/her social and environmental resources. Furthermore, an evaluation will be made as to the extent of the client’s level of independence and/or need for home assistance to perform necessary ADLs. As well, a cost analysis detailing costs for replacement services and/or rehabilitative equipment to compensate for loss of independent functioning is provided (as appropriate). The noted information will be presented to the consumer in report format.

Home Safety & Independence Assessment

An occupational therapist will assess the client’s ability to perform his/her Activities of Daily Living (ADLs) including self-care tasks and instrumental ADLs taking into consideration the client’s physical and cognitive abilities as well as his/her social and environmental resources. Safety and independence at home is the focus of this assessment to optimize quality of living. This assessment will be individualized to meet the client’s specific needs and may include completion of various standardized assessment(s) including AMPS, MoCA, MMSE, as well as, verbal reasoning, telephone skills, money management, picture interpretation in combination with the functional in-home assessment of cognitive/physical abilities. Strategies and/or techniques for enhanced function and equipment recommendations will be provided with report and/or an in-person meeting with the family as requested.

Home Accessibility Assessment & Modification

An occupational therapist will determine the overall accessibility of the home environment and identify obstacles to optimal performance in function and mobility. As well, the occupational therapist will report on priority areas to improve accessibility or safety and initiate contact with appropriate resources for installation of adaptive equipment and aids. Functional Advances Inc. uses assessments and modifications in accordance with Canada Mortgage and Housing Corporation (CMHC) as well as Health Canada Guidelines.
